How it Works

Any current BioMedix™ client, physician or industry partner can earn a free Apple^® iPad^® 2 for every 2 referrals that become new BioMedix™ clients. Simply submit your interested colleagues’ contact information and BioMedix™ will alert you when you are eligible to claim your iPad^® 2.

How To Participate

1. Submit referral information using the online form to the right, email the information to Sales@BioMedix.com or call 877-854-0014 for more information. Referrals will be accepted until December 31, 2011.

2. BioMedix™ will alert you if your referral is eligible for the program.

3. Two of your referrals decide to purchase one of the products listed below.

4. You receive notification when you are eligible to claim your iPad^® 2!

Terms and Conditions

Participation in the BioMedix™ 2-For-2 Program (“the Program”) is subject to the following Terms and Conditions (“Terms and Conditions”). BioMedix™ reserves the right to cancel, suspend and/or modify the Program, or any part of it, at any time at its sole discretion. You (“the participant”) should carefully review these Terms and Conditions, as your participation in the Program implies your agreement to be bound by them. If you do not agree to these Terms and Conditions, you are not eligible for participation in the Program.

I. PRIVACY. Your privacy is important to BioMedix™. BioMedix™ may use the personal information you submit here to: send you special offers and email alerts in relation to the 2-For-2 Program, contact you for further information, ship any rewards to you, send special offers and email alerts to your referrals and contact your referrals for further information. In addition to the disclosures reasonably necessary for the purposes identified elsewhere above, BioMedix™ may disclose your personal information to the extent that it is required to do so by law, in connection with any legal proceedings or prospective legal proceedings, and in order to establish, exercise or defend its legal rights. BioMedix™ will take reasonable technical and organizational precautions to prevent the loss, misuse or alteration of your personal information.

II. PROGRAM PARTICIPATION/ELIGBILITY. The Program is only open to current BioMedix™ clients, physicians and industry partners. Subject to all applicable federal, state, provincial, territorial, municipal and local laws and regulations. Void where prohibited. Referrals must be complete, usable information, be submitted on or before December 31, 2011 at 11:59pm CT, and at least two (2) must result in a purchase in order to qualify for any reward. Any referrals that have been previously submitted or are already BioMedix™ contacts or clients will not be eligible. BioMedix™ retains the right to end the Program early at any time without notice. Participants can submit referrals using one of the following accepted methods: using the online submission form, emailing complete information to Sales@BioMedix.com, faxing or emailing a paper referral form. Any other methods of submission and any incomplete or illegible submissions are subject to disqualification at the discretion of BioMedix™. Referrals can be submitted separately and accrue, or be submitted in pairs. There is no limit on referrals submitted per participant (BioMedix™ reserves the right to alter this rule at any time).

III. REWARDS. When the required number of referrals resulting in new BioMedix™ clients is reached, an email alert will be sent, and the reward will be shipped within 2 - 3 weeks of eligibility to the address provided. BioMedix™ does not guarantee shipping times or methods, and is not responsible for lost, stolen, damaged or misdirected packages. This program does not imply any sponsorship by or affiliation with Apple^® or iPad^®. All product specifications for any Apple^® iPad^® 2 provided to an eligible healthcare provider pursuant to this program shall be determined by BioMedix™ at its sole discretion. The recipient shall be responsible for any charges, fees or other costs associated with internet service, data plans, downloads or any other additional services or features offered in connection with the iPad^® 2 by Apple® or any other entity. BioMedix™ reserves the right to substitute rewards at any time. Rewards are available while supplies last and on a first-come first-served basis.
